The change can be stark. People who always enquired how things were when it was a case of serious illness, suddenly have nothing at all to say, when the illness ends in death, as though they daren't even say 'How are you?' any more. Not everyone of course, and as you note it's somehow more understandable if someone is very young, but it's hard to fathom.
